Everytime I've had ER positive HER2 negative cancers that were operable surgery was the first step followed by chemo and/or radiation and then drugs if necessary. This includes my 1st and 2nd bouts with breast cancer and the original endometrial cancer. For the 1st breast cancer I had a double mastectomy and that was it, no drugs, radiation, or chemo. My 2nd round of breast cancer I had 3 surgeries, followed by 8 rounds of chemo, followed by 20 rounds of radiation, and now I'm on Letrozole and Ibrance. For the original endometrial cancer the first step was surgery to remove my uterus, ovaries, tubes, and cervix which was followed by 7 sessions of brachytherapy radiation. My 1st endometrial cancer recurrence was inoperable. It was like my breast cancers in that it was ER positive and HER2 negative. For this recurrence I was given an AI for a few months to see if it would slow and reverse the tumor growth, but it did nothing other than slow it down a teeny tiny bit. The next step was chemo, but it didn't kill this particular cancer. The final step was radiation which did kill this tumor. The 2nd and 3rd endometrial cancer recurrences were small lymph nodes that were treated and killed with 5 radiation sessions over the course of 1 week.
