I personally feel that I would have liked immediate implants. Large breasts where never my choice anyway. I just wanted some definition in the area. I still have a D cup and would have been happy with something even smaller then that. When I think back now I could have done without the fills and everything else that went with it.

I did not exactly do this, but my experience might be helpful. I had expanders put in. I planned to have them inflated slightly, but not to my original size. After a few weeks, I had an issue with one of the expanders. We thought it was an infection, but it turned out to be the incision was irritated by the hard edge of the expander. My plastic surgeon wanted to replace the expander with another one, see if it took and then proceed with the fillings. I asked him to replace it with a permanent implant. My way of thinking was, if it took, then it was one less surgery I needed to have. By this time I was really annoyed and tired. I wanted the surgery & healing to end, so I could start Chemo and move on. My plastic surgeon had never done this, but said it was worth a shot If I did not care about the small breast. It took, I was right, One less surgery and I was spared the fills. I love my small breast! I tell everyone I spent the first half of my life with breast too big for my frame and thought I would try to other way around. Small is way better! I have no regrets and no tightened feeling either. Hope that helps. Good luck. YOur decision will be what is best for you.

My friend did this 12 weeks ago. Had second surgery this week to fix it. You have to be pretty small breasted for this to be an option though. Personally I wonder if not expanding first will create a tightened feeling you can never get rid of.

I went direct to implant, no expanders & so did my sister (we are both BRCA 1). Hers was prophylactic & hasn't had many complaints. Me on the other hand, I hate them! The allograft stetched so one implant is lower than the other and goes into my armpit a little. It is very very annoying! My plastic surgeon has discussed doing a revision where he will swap the one implant with a different shaped one. Even though he says I really won't experience pain, I'm still thinking about it because I am pretty tired of being poked after the mastectomy, oopherectomy, chemo etc.
