It depends. Generally, for lymph node positive, triple negative breast cancer, inflammatory breast cancer and Her2+ with a larger tumor, Neo adjuvant (chemo before surgery is recommended. For any invasive breast cancers that are hormone positive, an Onco Type test is done on the surgical pathology to determine if chemo should be done, Her2+ with a smaller tumor or if a sentinel lymph node is tested in surgery and it is lymph node positive, adjuvant chemo, chemo after surgery may be recommended. Radiation is usually last.

I was diagnosed with invasive duct carcinoma necrosis at stage 3b. I had my lumpectomy first, chemo, lymphnode removal, then radiation!

I have IDC stage 1 - had surgery first then chemo. My oncotype score came out as high risk.

Mine was 16 rounds of chemo with 8 rounds of keytruda then MRI showing tumor gone then surgery and continue with 9 more rounds of Keytruda then reconstruction
