I've had multiple expander implants, saline implants, and gel implants and the gel are by far the most comfortable implants to live with from a physical and wearability standpoint. Expanders are just plain old uncomfortable. To me the saline feel heavier, they are harder, and they don't look good a lot of the time. The gel feel lighter to me, are softer and more comfortable feeling in my chest, and they tend to look better both dressed and undressed.

Both saline and silicone implants are considered safe options after mastectomy β€” neither is definitively "healthier" than the other. The best choice really depends on your body, your goals, and your personal priorities.

Here's a quick comparison to help think it through:

Silicone Implants
- Look and feel more like natural breast tissue
- A rupture can be "silent" β€” meaning you may not notice it right away
- Require an MRI or ultrasound 5–6 years after surgery, then every 2–3 years to check for leaks
- Studies show a silicone rupture does not cause serious health problems

Saline Implants
- Feel firmer than silicone
- A rupture is immediately obvious β€” the breast visibly deflates within hours or days
- No extra imaging needed after placement
- The saltwater is safely absorbed by the body if a rupture occurs One MyBCTeam member chose saline specifically because a rupture would be immediately noticeable, which gave them peace of mind. Others love silicone for its natural look and feel.

Both implant types carry about a 1% chance of rupturing per year. The most important thing is having an open conversation with your surgeon about why they're recommending silicone β€” there may be specific reasons related to your body or treatment plan that make it the better fit for you.
